| How do Students Qualify
for Academic Accommodations? |

To establish eligibility for academic accommodations the student must present legible and professional documentation of the disability to a disability counselor. The following are guidelines for necessary documentation based on the student’s area of disability.

| Learning Disability/ADHD/ADD |
- testing administered within the last three years by a licensed professional in the area of requested disability accommodation
- a narrative report including a summary of the administered testing
- specific statement of need for accommodations based on testing results
|
| Medical Disability |
- a statement of diagnosis from a medical doctor including
need for accommodations
- a specific time interval for needed accommodations if a temporary disability
|
| Psychiatric Disability |
- a statement of diagnosis from a qualified professional
- a brief report related to the diagnosed disability and recommended accommodations
|
| Recordings for the Blind and Dyslexic |
