To establish eligibility for academic accommodations the student must present legible and professional documentation of the disability to a disability counselor. The following are guidelines for necessary documentation based on the student’s area of disability.

• testing administered within the last three years by a licensed professional in the area of requested disability accommodation
• a narrative report including a summary of the administered testing
• specific statement of need for accommodations based on testing results

• a statement of diagnosis from a medical doctor including need for accommodations
• a specific time interval for needed accommodations if a temporary disability

• a statement of diagnosis from a qualified professional
• a brief report related to the diagnosed disability and recommended accommodations

• Procedures for requesting textbooks
• Textbook request form
