Aptitude/Cognitive Ability (intellectual
assessment: WAIS-4)

Academic achievement (
academic functioning: SATA or WIAT - 2 or Woodcock Johnson - WJ III )

Information Processing (
subtests on WAIS - 4 or Woodcock Johnson - WJ III)
- Personality test (preferred to rule out DSM-IV-TR diagnoses)
- Documentation must include a specific diagnosis from the
DSM-IV-TR

Recommended accommodations by the evaluator
should include rationale

- Must have been completed within the past three years
- Evidence of early impairment (academic and social history)
- Diagnostic interview: information from a variety of sources
- Assessment: neuropsychological or psychoeducational evaluation
- Aptitude/Cognitive Ability (intellectual assessment: WAIS-4)
- Academic achievement (academic functioning: SATA or WIAT-2 or Woodcock Johnson - WJ III )
- Behavioral rating scales of AD/HD
- Personality test (MMPI-2) Rationale: rule out alternative
diagnosis which could include mood, behavioral, neurological,
and/or personality disorders.
- Documentation must include a specific diagnosis from DSM-IV-TR.
- Recommended accommodations by the evaluator should include
rationale.
