Requirements for Proper Documentation

Evaluation Process
Students will receive an initial interview with a disability professional to review documentation of their disability and assure compliance with Section 504 of the Rehabilitation Act of 1973.

Documentation Requirements

Learning Disability
Testing must have been completed within the past three years and must be comprehensive, not relying on any one test or subtest. We will need a neuropsychological or psychoeducational evaluation, which includes:
  • Aptitude/cognitive ability (intellectual assessment: WAIS–4)
  • Academic achievement (academic functioning: SATA or WIAT–2 or Woodcock Johnson–WJ III)
  • Information processing (subtests on WAIS–4 or Woodcock Johnson–WJ III)
  • Personality test preferred (to rule out DSM-IV-TR diagnoses)
  • Documentation must include a specific diagnosis from DSM-IV-TR
  • Recommended accommodations by the evaluator, including rationale
AD/HD
Testing must have been completed within the past three years. Documentation should include:
  • Evidence of early impairment (academic and social history)
  • Diagnostic interview with information from a variety of sources
  • Neuropsychological or psychoeducational evaluation, including:
  • Aptitude/Cognitive Ability (intellectual assessment: WAIS–4)
  • Academic achievement (academic functioning: SATA or WIAT–2 or Woodcock Johnson–WJ III)
  • Behavioral rating scales of AD/HD
  • Personality test (MMPI–2) to rule out alternative diagnosis, which could include mood, behavioral, neurological and/or personality disorders
  • Documentation must include a specific diagnosis from DSM-IV-TR
  • Recommended accommodations by the evaluator, including rationale
Medical Disability
  • Statement of diagnosis from the treating medical professional, including need for accommodations
  • Statement of how the disability limits the student in an academic environment
  • Specific time interval for needed accommodations, if temporary
Psychiatric Disability
  • Statement of diagnosis from a qualified treating professional
  • Statement of how the disability limits the student in an academic environment
  • Brief report related to the diagnosed disability and recommended accommodations
