Alcohol Skills Training Registration Form

(Please understand that registration for the Alcohol Skill Training can also be conducted in person in 110 Moody Hall. Online registration is not a secure means of communication. Therefore, confidentiality cannot be guaranteed. However, all information provided will be held in confidence with exceptions provided by state and federal law.)

Location : 306 Fleck Hall (except where indicated)
Time: Promptly starts at 2:00 PM
Duration: 120 minutes
Size : Limited to no more than nine students
Cancellation Policy : Four or fewer will be rescheduled to next session

By submitting the Alcohol Skills Training Registration form, you are acknowledging you understand the following:

  • The Counseling & Consultation Center staff has the student's permission to notify the Conduct Officer that the student made their appointment(s) and/or fulfilled their tasks as indicated above. The student also gives the Counseling & Consultation Center staff permission to notify the Conduct Officer if significant indicator(s) suggest the student will need additional assessment and/or therapeutic services, which may be beyond the scope of what is offered at the university. At which time, the Conduct Officer will provide appropriate referral resources and, if determined to be relevant to circumstances of the offense, develop a behavioral contract to ensure the well being of the student, the community, and the student's requirement to fulfill the educational intent of the sanction requirements.
  • The student agrees to satisfy the requirements outlined above to fulfill the sanctions related to violation(s) of the Student Code of Conduct and any amendments. The student has agreed to complete each component to satisfy the sanction. The student has provided the Counseling & Consultation Center staff informed consent to notify specific departments about their attendance and fulfilled tasks. By clicking the submit button below, the student is affirming that they have received information and instructions, fully understand the program requirements, and have had all questions answered related to the responsibilities needed to complete the Alcohol & Other Drug Education program.
