For easy reference, locate the descriptor which most closely describes your activity in the left hand column. Then, follow across the chart to the right to see which forms, indicated by the , will need to be filled out for that activity.

For estimating level of risk, use the following guideline:
Low: may cause minor injury, illness, property damage,
financial loss or negative publicity for SEU
Medium to High: can range from severe injuries to death

Internships refer to assignments for college credit that involve supervised, practical working experience, either paid or unpaid. A copy of an Internship information sheet and learning agreement should be sent to the Office of Career Planning and Experiential Learning Office in Moody Hall 134, to the attention of David Uribe.

Service-Learning is a academic part of credit-bearing classes and is a method by which "students learn and develop through thoughtfully-organized service that: is conducted in and meets the needs of a community and is coordinated with an institution of higher education, and with the community; helps foster civic responsibility; is integrated into and enhances the academic curriculum of the students enrolled; and includes structured time for students to reflect on the service experience" (American Association for Higher Education (AAHE): Series on Service-Learning in the Disciplines (adapted from the National and Community Service Trust Act of 1993). Please send a copy of the Learning Agreement for Service-Learning to Jack Green Musselman, CTE, campus mail 1067.

Forms should be kept in a designated location in your department for a calendar year after your event.

Please note that requests for authorization need to be submitted in advance to your Dean or Department Head to approve the trip.

Student travel within 25 miles of campus does not have to be regulated by use of risk management forms. However, faculty and staff members should not transport students in their personal vehicles or organize carpools. Students who wish to carpool to campus activities should make arrangements for their own transportation.

University sponsored student travel farther than 25 miles from campus should be done by university van or rental vans driven by SEU approved drivers over the age of 21 who have taken the physical plant driving test.
