STAFFORD
LOAN PROMISSORY NOTE |
Stafford loan borrowers (undergraduates and graduates)
who are new to St. Edward's must complete a Stafford Loan
Master Promissory Note, even if they have borrowed a Stafford
Loan previously at another institution. The Master Promissory
Note will be included in your financial assistance packet.
However, if you have misplaced the form or need another,
you can access the application below or request that another
application be sent to you.

| STAFFORD
LOAN ACCEPTANCE FORM |
All Stafford loan borrowers (undergraduates and graduates)
must submit a Stafford Loan Acceptance Form each academic
year indicating the amount and type of Stafford loan they
are accepting. Students requesting additional loan funds
during the academic year must also submit an acceptance
form for the increased amounts. The Acceptance Form is
not
the same form as the Stafford Loan Master Promissory Note.
The Aid Acceptance Letter included in your financing packet
serves as the Loan Acceptance Form. If you have misplaced
your Aid Acceptance Letter, you can e-mail
Student
Financial Services to have a duplicate copy of your Aid
Acceptance Letter mailed or faxed to you. You can also
complete your acceptance on-line via your EdWeb Account.
To submit an on-line acceptance: First,
login to your EdWeb account.
Next, select the "Financial Aid" link in the
left-hand frame. Then, select the "Aid Awarded" link
in the bottom frame (make sure the correct award year
is selected--if not, change year and click "View financial
aid year" button). Finally, follow on the on-line instruction
to complete your acceptance.

| PERKINS
LOAN PROMISSORY NOTE |
| A Master Promissory note is required for all borrowers
who have been awarded a Federal Perkins Loan as part of their
financial aid package at St. Edward’s University.
The Master Promissory Note will be included in your financing
packet. However, if you have misplaced the form or need another,
you can access the application below or request that another
application be sent to you.

| EDPAY
PACKET |
Students who receive employer reimbursement for their
tuition at the end of the semester can use St. Edward's
Employer Deferred Payment (EDPAY) Program to finance their
semester charges. Students must complete an enrollment package
to participate in this program.

| PLUS
LOAN REQUEST FORM (for
previous parent loan borrowers at SEU) |
| Parents who have previously borrowed a
PLUS loan at St. Edward's should complete this form to request
an additional parent loan for the new academic year. The
loan will be processed through the lender indicated on the
parent's PLUS Master Promissory Note on-file with the university.

| SATISFACTORY
PROGRESS FINANCIAL AID APPEAL FORM |
Students who have been denied financial assistance for
failing to meet the university's satisfactory academic
progress
requirements may appeal that decision by completing and
submitting this appeal form to
the Progress Appeals Committee.
The committee considers appeals based upon mitigating/extenuating
circumstances. Submission of an appeal form does not guarantee
approval by the committee.

FOR SCHEDULE ADJUSTMENT/REFUND REQUEST |
Students who wish to petition for an adjustment to their
course schedule and/or request a refund past the university's
published deadlines must complete and submit this form.

FORM |
Students who participate in the university's
Monthly Arrangement Payment Plan (MAPP) can complete

REMISSION ENROLLMENT FORM |
Qualified St. Edward's University employees seeking
tuition remission for themselves or their dependent must
complete
and submit
this form to the Office of Student Financial Services at
least thirty days before the start of each semester.

| TUITION EXCHANGE APPLICATION |
Tuition Exchange is open to children of full-time regular employees who have at least 3 years continuous service. Children interested in participating in this benefit during the Fall semester must make application to St. Edward's Office of Student Financial Services by December 1st of the preceding year.
Given the very limited number of slots available each
year, participants will be selected based on their parents'
regular seniority. Eligibility for the program ceases at
the end of a semester in which the employee separates service
or changes to a non-benefits eligible status.
