A continuing student is a student who goes every term without a break of more than one semester. If this is the case, all that is needed from the student is to fill out an enrollment certification request. This form an be filled out online and transmitted to this office or the student can obtain the form at the Veterans Affairs Office.

More than one semester since last attendance.
Under these circumstances a VA Form 22-1995 or 22-5495 is required along with the enrollment certification request. Both can be obtained online r from the Veterans Affairs Office.

Tutorial Forms
Eligible veterans who have a course deficiency may be eligible to receive reimbursement for payment to a tutor for individual tutoring, 22-1990t. The veteran, tutor, course instructor, and Veterans Certifying Officer must complete parts of the tutorial form.

Changes
Be sure to keep your Certifying Office informed of all changes in stature: change of address, name, drop/add activity, or change of major (here a new degree plan is required).
